    • 我们为HIV感染者提供形式多样的门诊保健服务，不同的服务接受情况会影响HIV感染的结局。研究对2008至2011年美国宾夕法尼亚费城瑞安诊所的HIV感染成人的保健护理进行了评估，比较了不同建制下（医院或社区基础的）患者HIV感染结局的差异。混合效果的Logisitic回归分析显示，在12637名患者中，共计32515人年，在25家诊所接受过HIV相关的保健服务（其中12家为医院基础的）。结果表明，女性，非西班牙裔黑人，有个人保险，家庭收入高者会更多的选择医院基础的诊所（p＜0.05）。在调整了患者及诊所因素情况后，不同建制下HIV患者的感染结局没有差异（调整OR值为1.24，95%CI为0.84-1.84）。总的来说，医院和社区基础的人群的人口学特征存在较大差异。
