    Abstract

Background: The ongoing outbreak of 2019 novel coronavirus disease (COVID-19) is a global threat. Clarifying its clinical features such as risk factors for disease progression is a pressing issue. We analyzed the difference between non-severe and severe cases with COVID-19 as a result of the mass infection on a cruise ship “Diamond Princess” in Japan.

Methods: In this retrospective, single-center study, total 104 cases of laboratory-confirmed COVID-19 were enrolled from the mass infection on the cruise ship from 11th to 25th Feb, 2020, at Self-Defense Forces Central Hospital in Japan. Clinical records, laboratory data, and radiological findings were collected and analyzed. Clinical outcomes were followed up until 26 th Feb, 2020.

Identification of markers for symptom onset and disease progression is a pressing issue. We described the clinical features of people infected on board the Diamond Princess cruise ship who were diagnosed with asymptomatic severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection or mild or severe COVID-19, on admission to the Self-Defense Forces Central Hospital (Tokyo, Japan) and at the end of observation. Methods This retrospective, single-centre study included participants with laboratory-detected SARS-CoV-2 infection who were admitted to the Self-Defense Forces Central Hospital from Feb 11 to Feb 25, 2020. Clinical records, laboratory data, and radiological findings were analysed. Clinical outcomes were followed up until discharge or Feb 26, 2020, whichever came first. We defined asymptomatic infection as SARS-CoV-2 infection with no history of clinical signs and symptoms, severe COVID-19 as clinical symptoms of pneumonia (dyspnoea, tachypnoea, peripheral capillary oxygen saturation <93%, and need for oxygen therapy), and mild COVID-19 as all other symptoms. Clinical features on admission were compared among patients with different disease severity, including asymptomatic infection, at the end of observation. We used univariable analysis to identify factors associated with symptomatic illness among asymptomatic people infected with SARS-CoV-2 and disease progression in patients with COVID-19.

    • Estimating the infection and case fatality ratio for COVID-19 using age-adjusted data from the outbreak on the Diamond Princess cruise ship Timothy W Russell, Joel Hellewell, Christopher I Jarvis, Kevin van-Zandvoort, Sam Abbott, Ruwan Ratnayake, CMMID nCov working group, Stefan Flasche, Rosalind M Eggo, Adam J Kucharski doi: https://doi.org/10.1101/2020.03.05.20031773 Abstract Adjusting for delay from confirmation-to-death, we estimated case and infection fatality ratios (CFR, IFR) for COVID-19 on the Diamond Princess ship as 1.2% (0.38-2.7%) and 2.3% (0.75%-5.3%). Comparing deaths onboard with expected deaths based on naive CFR estimates using China data, we estimate IFR and CFR in China to be 0.5% (95% CI: 0.2-1.2%) and 1.1% (95% CI: 0.3-2.4%) respectively. *注,本文为预印本论文手稿,是未经同行评审的初步报告,其观点仅供科研同行交流,并不是结论性内容,请使用者谨慎使用.
