    Abstract

    Background: Since January 23rd 2020, stringent measures for controlling the novel coronavirus epidemics have been gradually enforced and strengthened in mainland China. The detection and diagnosis have been improved as well. However, the daily reported cases staying in a high level make the epidemics trend prediction difficult.

    Methods: Since the traditional SEIR model does not evaluate the effectiveness of control strategies, a novel model in line with the current epidemics process and control measures was proposed, utilizing multisource datasets including cumulative number of reported, death, quarantined and suspected cases. Results show that the trend of the epidemics mainly depends on quarantined and suspected cases.

    Findings: The predicted cumulative numbers of quarantined and suspected cases nearly reached static states and their inflection points have already been achieved, with the epidemics peak coming soon. The estimated effective reproduction numbers using model-free and model-based methods are decreasing, as well as new infections, while new reported cases are increasing.

    Interpretation: Most infected cases have been quarantined or put in suspected class, which has been ignored in existing models. Further, uncertainty analyses reveal that the epidemics is still uncertain and it is important to continue enhancing the quarantine and isolation strategy and improving the detection rate in mainland China.

The number of confirmed infected individuals are still increasing rapidly. Estimating the accurate infected population and the future trend of epidemic spreading under control measures is significant and urgent. However, the common forecasting models, such as SI, SIS, SIR, SIRS and SEIR, are only suit for scenarios without non-pharmaceutical prevention interventions. And the estimating infected populations from existing literature are too far more than the official reported data. Here, we provide a two-phase EI model integrated the epidemic spreading before and after control measures. Then, we estimate of the size of the epidemic and simulate the future development of the epidemics under strong prevention interventions. Methods: According to the spread characters of 2019-nCov, we construct a novel exposed-infected (EI) compartment system dynamics model. This model integrates two phases of the epidemic spreading: before intervention and after intervention. We assume that 2019-nCov is firstly spread without intervention then the government started to take strong quarantine measures. Use the latest reported data from National Health Commission of the People’s Republic of China, we estimate the basic parameters of the model and the basic reproduction number of 2019-nCov. Then, based on this model, we simulate the future spread of the epidemics. Both the infected population and the development time of 2019-nCov under different prevention policy scenarios are estimated. And, the influences of the quarantine rate and the intervention time point of prevention intervention policy are analyzed and compared.

We speculated that the emigration rate of Wuhan population might have an important impact on the number of confirmed cases and mortality in other areas of Hubei Province. Methods: Based on the official website of health commission of Hubei Province (wjw.hubei.gov.cn), we obtained the trend of pneumonia epidemic with novel coronavirus infection over time in Hubei Province. By analyzing Baidu map migration big data (qianxi.baidu.com),we also got the flow direction of Wuhan population from January 20,2020 to February 7,2020. Results: Since January 20, 2020, the number of confirmed cases, cured cases and dead cases of novel pneumonia in Hubei Province has been increasing. However, after February 4, 2020, the number of cured cases has exceeded the number of dead cases. The case fatality rate gradually increased from January 20 to January 24, decreased on January 25, reached the peak on January 26, then gradually decreased, and finally stabilized at about 2.8. The results of correlation analysis showed that the average emigration rate in Wuhan was positively correlated with the number of confirmed cases (r=0.899, p<0.001) and the number of fatalities(r=0.851, p<0.001). Conclusions: China is still facing a huge challenge of novel coronavirus. However, despite the increasing number of confirmed cases, the current cure rate has exceeded the case fatality rate. There is a positive correlation between the emigration rate of Wuhan population and the number of confirmed cases and the mortality rate in Hubei Province, so we still need to strictly control the outward circulation of Wuhan population.
