    Reports of coronavirus disease 2019 (COVID-19) show that children usually have mild clinical signs and less severe disease than do adults,1 although the reasons for such differences are not yet completely understood.2 A previous report3 describing paediatric COVID-19 cases in China showed that 13 (0·6%) of 2135 laboratory-confirmed and suspected cases were critical, but myocardial involvement was not noted. At the time of writing, two clinical scenarios of severe paediatric COVID-19 cases have been reported from the UK and Italy: severe shock syndrome with hyperinflammation4 and Kawasaki-like-disease.5

Show all authors Published:May 19, 2020DOI:https://doi.org/10.1016/S2352-4642(20)30165-6 We report eight children with COVID-19 presenting at a single centre in the UK with symptoms of atypical appendicitis before rapid deterioration requiring hospitalisation and, in some cases, intensive care support. All children had imaging confirming terminal ileitis and no surgical intervention was required at the time of writing. We draw attention to an unusual presentation of COVID-19 in children and adolescents and we recommend abdominal imaging when investigating for possible appendicitis. There have been 4?730?968 confirmed cases and 315?488 deaths from COVID-19 worldwide since emergence of the infection in December, 2019, in Wuhan, China.1 To date, most of the case load and mortality has been seen in the adult population. Currently in the UK, there is concern regarding an inflammatory syndrome related to COVID-19 in children with gastrointestinal symptoms in the presence of both positive and negative severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) PCR tests.2, 3

This review characterizes clinical symptoms, laboratory, and imaging findings, as well as therapies provided to confirmed pediatric cases of COVID-19. Methods Adhering to PRISMA guidelines, we searched four medical databases (PubMed, LitCovid, Scopus, WHO COVID-19 database) between December 1, 2019 to May 14, 2020 using the keywords “novel coronavirus”, “COVID-19” or “SARS-CoV-2”. We included published or in press peer-reviewed cross-sectional, case series, and case reports providing clinical signs, imaging findings, and/or laboratory results of pediatric patients who were positive for COVID-19. Risk of bias was appraised through the quality assessment tool published by the National Institutes of Health. PROSPERO registration # CRD42020182261.
